Step 1
Cook penne pasta in a large pot of generously salted boiling water according to package directions until al dente. Reserve about 1 cup of pasta water before draining.
Step 2
While pasta cooks, heat olive oil in a large skillet or Dutch oven over medium heat. Add diced onion and cook until softened, about 5 minutes. Add minced garlic and red pepper flakes (if using) and cook for 1 minute until fragrant.
Step 3
Pour in vodka and bring to a simmer, scraping up any browned bits. Cook for 2-3 minutes to burn off the alcohol.
Step 4
Stir in crushed tomatoes. Bring to a gentle simmer, then reduce heat and cook for 10-15 minutes, allowing flavors to meld.
Step 5
Stir in heavy cream and mix until fully combined and the sauce turns a beautiful pink color. Season with salt and pepper to taste.
Step 6
Add the drained penne pasta to the sauce. Toss to coat, adding a splash of reserved pasta water if needed to reach desired consistency. Serve immediately, garnished with grated Parmesan and fresh parsley.
